Medicaid waiver Application: Home and Community Based Services (HCBS)
The Medicaid Service Coordinators (MSC’S) help the individuals with intellectualdisability and their families/advocates in applying for Medicaid sponsored services to enable them start enjoying Medicaid and OPWDD services.
Intake
The MSC’s assist the disabled individuals and their families/advocates to obtain Waiver services through front door application. The MSC work with the intake coordinators at OPWDD in support of the individuals after making referrals for the front door session. With the Front door approval, the individuals will have the opportunity to pursue their heart desires and choice of endeavors in the communities.
SERVICE COORDINATION/PLAN OF CARE SUPPORT SERVICES (PCSS)
The MSC’s help in the organization and coordinate the activities of the individuals including making referrals for medicals assessments, Psychological and psychosocial evaluations, for recreational and education services, counseling services and other emergency situations as needed by the individuals. We also assist the individual to obtain Residential and Day habilitation. Most essentially, our MSCs make follow ups to ensure that all referrals are duly executed and through.
MSCs also develops, reviews and monitors the individualized Service Plan (ISP) to reflect the varying needs, choices and favorites of the individual that is often requested by the individuals, parents and advocates. The MSC’s in coordination of the individualized Service Plan (ISP) help in the plan of care by assessing and updating significant or noteworthy changes in their service plans.
FAMILY REIMBURSEMENT
The MSC department helps with the application for family reimbursement for those individuals/families who spent monies for their persons with special needs to attend paid respite programs, summer camps and other activities in the communities to socialize and have fun with their peers.
SELF DIRECTED SERVICES
The Medicaid service coordinators assist the individuals through OPWDD to maximize their needs for self-directed services to enable them live a productive and enjoyable lives. With these services, the individuals based on their self-centered goals choose to live in their own apartment where the MSC assist in the application for the Individualized supports and services (ISS) to get their own apartment. This increase the independence of the individuals involve.
In addition to this service, the individual upon approval can make an essential decision to hire a support staff to assist them in their daily activities.
APPLICATION FOR FINACIAL AND OTHERS SUPPORTS
MSCs aid in applying and maintaining financial status like Supplemental Security Income (SSI) and Social Security (SS), and in addition to foodstamps application as well as legal and medical welfares, connection to health care services, instructions and guidance in management of activities of daily living skills,
